I have Providence Choice, if I only go to “In-Network” doctors and facilities, do I still need to inform Providence of my Medical Home?

Yes, you must inform Providence of your Medical Home.  Otherwise, all services will be treated as out-of-network even if you see In-Network providers.  You can notify Providence of your Medical Home online at myProvidence (register for an account if you don’t have one), by submitting the Medical Home Selection Form, or by calling Providence Customer Service at 503.574.7500 or 800.423.9470.
