Prescriptions Filled at OSU Student Health Services Pharmacy

Gym Subsidy - $15 per month

  • Effective October 1, 2013
  • Informational brochure
  • Employee and enrolled spouse/domestic partner may participate in this program and receive the $15 per month subsidy.
  • Must workout at least 12 times per month to receive the subsidy for that month. Members will receive credit for one workout session per day, with at least 8 hours between sessions.
  • Search for “In Network” facilities at Fitness Facilities. OSU Recreational Sports and the Faculty Staff Fitness Program are approved facilities but not “In Network”. Therefore, they will not be listed on this report.
  • Register online at www.exerciserewards.com.
  • The deadline to submit for reimbursement is 90 days from the end of the plan year. Since this is the first year of the program, employees have until March 31, 2015 to submit forms for the time period of October 2013 – December 2014. 
  • OSU Dixon Recreation Center - Recreational Sports is an “APPROVED” program.
    • Recreational Sports website
    • Complete the Fitness Facility Member Verification Form.  To be submitted once per year with the first request for reimbursement.
    • Print a copy of your Member Visit Report from Recreational Sports’ Web Track system
    • CompletetheReward Submission Form/Log 
    • Mail completed documents to Exercise Rewards Program for reimbursement.  Documents to be submitted are: 1) Fitness Facility Member Verification Form (once per year); 2) Reward Submission Form/Log; and 3) Web Track Member Visit Report
  • OSU Faculty Staff Fitness Program is an “APPROVED” program.
    • Faculty Staff Fitness website
    • Complete the Fitness Facility Member Verification Form.  To be submitted once per year with the first request for reimbursement.
    • Complete the Reward Submission Form/Log and submit to Faculty Staff Fitness Office at the end of each quarter for review and verification.
    • Mail completed forms to Exercise Rewards Program for reimbursement.  Forms to be submitted are: 1) Fitness Facility Member Verification Form (once per year); and 2) Reward Submission Form/Log.
    • If your class meets twice a week, you will also need to workout at either an “Approved” or “In Network” gym to meet the 12 times per month requirement.
  • IN NETWORK” facility –
    • Show the facility the ExerciseRewards ID card found in the Exercise Rewards Program brochure
    • The facility will submit your exercise sessions directly to ExerciseRewards each month. 
    • You will automatically receive your rewards check (approximately 30 days after the close of the calendar quarter) for each month you worked out at least 12 times.

Healthy Team, Healthy U:

  • This is a 12-week team based wellness program with proven results for losing weight, increasing fitness, and lowering blood pressure.
  • Employees enrolled in a PEBB Health plan and covered family members age 18 or older are eligible to participate in this program.
  • Team up with co-workers and complete the 12-week program.  The 30-minute sessions would be completed during “non-work” times (e.g., breaks, before/after work, during lunch).
  • The program can also be completed independently or by teaming up with family members.

MoodHelper

The Kaiser Permanente Center for Health Research is looking for 500 PEBB volunteers to pilot this online program for managing depression. You do not need to be a Kaiser member. Program begins October 1, 2013.

Living Well with Chronic Conditions and Diabetes Prevention

These are tested, proven programs and will be available at no cost to PEBB members in a variety of locations around the state. Start times vary. Additional information will be posted when available.
