Welcome to the 2026 Benefits Open Enrollment page! Here, you will find all the information you need to make informed decisions about your benefits for the upcoming year. Our comprehensive guide includes everything from plan changes and help sessions to resources and wellness programs.

Open Enrollment Summary & 2026 Plan Year Changes

PEBB open enrollment is October 1-31, 2025, and is mandatory this year for all PEBB members. The changes you make during Open Enrollment will be effective in the new plan year beginning January 1, 2026.

Open Enrollment is your once-a-year opportunity to make changes to your benefit packages without having experienced a qualifying life event or family status change (marriage, birth, divorce, loss/gain of other insurance, etc.). 

What does that mean for you?

It means you must login to your PEBB portal to complete Open Enrollment, even if you will be opting out or declining coverage. Completing this includes:

  • Reviewing your current enrollments
  • Changing any of your coverages
  • Adding eligible dependents or removing eligible dependents
  • Enrolling or re-enrolling in a Flexible Spending Account (FSA)
  • Updating your personal information
  • Updating your beneficiaries

By not completing Open Enrollment this year, the following will occur:

  • Your existing medical, dental, and vision selections will remain the same and cannot be changed until October 1, 2026 (outside of a qualifying life event*).
  • PEBB surcharges will be subtracted from your paycheck during the 2026 calendar year.
  • You cannot contribute to a Flexible Spending Account (FSA). You must enroll or re-enroll each year to participate in an FSA. 

How do you complete open enrollment?

Employees can enroll online or use paper forms. 

Go to: www.Pebbenroll.com to complete this process online.

Paper forms can be faxed, scanned and e-mailed or sent through mail.

*Life Events Outside of Open Enrollment: If you incur or anticipate incurring a qualifying life event during Open Enrollment such as birth, marriage, divorce, please contact Employee Benefits at 541-737-2805 or [email protected] in order to complete the Midyear Change process prior to completing Open Enrollment. If you incur a qualifying life event during the year, do not wait until Open Enrollment to update your elections. Contact Employee Benefits or refer to our website on how to update you benefit elections. Visit the Midyear Changes to Health & Optional Benefits page for additional information.

Dependent Eligibility (required)

PEBB will request verification of eligibility for dependents added during Open Enrollment and throughout 2025 for qualifying Midyear Changes (birth, marriage, etc.).

What does that mean? If you add dependents during Open Enrollment or through a Midyear Change Form during 2025, PEBB will send you a packet of information. PEBB will ask you to confirm that each of your dependents are eligible. To complete the process, you will simply:

  • Make copies of requested proof
  • Mail, fax, or email the above to the PEBB Dependent Eligibility team. (NOTE: Email is not a secure method for sending documents.)

Read more about dependent eligibility on the Oregon Health Authority website. Make sure you have everything you need. Don't send anything now; wait until you receive your letter from PEBB later this year. NOTE: If you do not respond to PEBB's letter, PEBB will remove the dependents added to your coverage.

Surcharges and Optional Life Rates

  • Tobacco Surcharge - $25/month if employee uses tobacco + $25/month if enrolled spouse/domestic partner uses tobacco
  • Spouse/Domestic Partner Surcharge - $50/month if spouse/domestic partner waives coverage in another employer’s group plan (non-PEBB plan) in favor of coverage through PEBB. 
  • Tobacco User Life Insurance Rates – The rates for optional employee life and optional spouse/domestic partner life insurance is higher for members that have used tobacco within the past 12 months
  • OEBB/PEBB double coverage Surcharge - You will have to pay a monthly $5 surcharge if you are an active full-time employee and:
    • Someone in your family is covered as a member under their own PEBB or Oregon Educators Benefit Board (OEBB) plan, and
    • That person is covered as a dependent (spouse, partner or child) on your PEBB medical plan.

 

Open Enrollment Corrections

All Open Enrollment Corrections are due by February 28, 2026

To make corrections by February 28, 2026, please fill out and return the Open Enrollment Corrections form to the PEBB Benefits team in University Human Resources at [email protected]

To make corrections for the Flexible Spending Accounts by December 12, 2025, please fill out and return the FSA Corrections form to the PEBB Benefits team in University Human Resources at [email protected] 

Remember to verify the December 2025 earnings statement to double check that your deduction section the PEBB enrollments that you selected.

2026 Plan Changes

Medical Plans

General/All Medical Carriers:

Health Engagement Model (HEM) for 2026: PEBB's wellbeing program, HEM, will end on December 31, 2025. PEBB will no longer offer the $17.50 monthly incentive, and the lower standard deductible amounts will be available to all members.

What does this mean? 

  • PEBB will no longer offer the $17.50 monthly incentive.
  • The lower standard deductible amounts will now be available to all members. This includes members who didn't participate in HEM in the past. 
  • You will no longer be asked to do a health risk assessment or wellness activities. However, these resources will still be available to you. 

Why is this changing?

  • PEBB will save money by ending HEM. The money saved helped ensure that the Board could keep costs lower for the 2026 medical plans.
  • In the future, the Board will look for a new wellness program that is easy to use and helps members stay healthier. 
Moda Plans
  • Out-of-area provider network: When a member is outside of the Moda Health service area, they'll now have access to Aetna PPO network.
  • Behavioral Health 360: Members will have access to new mental health support and services.
  • Nurseline: This service will no longer be available. TelaDoc and CirrusMD are available to provide similar care options.
  • ID cards: Members will receive new ID cards.
  • Partnering with Garner*: Garner helps members find high-quality care.
  • Mandatory pharmacy program*: For regular medication
Kaiser Plans
  • There are no plan changes to the Kaiser Permanente plans for 2026.
Providence Plans
  • Partnering with Collective Health: Together, their new name will be Providence Health Powered by Collective Health®. Collective Health will help Providence Health manage health insurance and medical care.
    • Collective Health will help Providence Health Plan manage health insurance and medical care.
    • They will take care of claims. 
    • They will also help you understand benefits and find the care you need.
      • It will be your "member advocate" and will help you ensure you have a good experience and get the right benefits without confusion.
  • No changes to coverage or the provider network. However, members will:
    • Use a new, dedicated member services phone number
    • Have access to an upgraded website and app.
    • Receive new ID cards
  • Partnering with Garner*: Garner helps members find high-quality care. 
  • Mandatory pharmacy program*: For regular medications

PEBB will offer the same dental and vision plan options. Plans are available through Delta Dental (Moda Health), Kaiser Permanente, and VSP. 

*Mandatory Pharmacy Program (Providence & Moda)

Providence Health Plan and Moda Health medical plan will have a new pharmacy program for your regular medications. 

  • Regular medications include drugs you take each day. They help treat and manage conditions like high blood pressure or high cholesterol.
  • This program is mandatory.
  • This new pharmacy program will save you money on your prescriptions. 
    • When you get a 90-day supply, it's cheaper than getting three prescriptions for 30 days each.
  • This new program also helps PEBB save money.
  • You'll have some time to make the transition to the mail-order program or a preferred pharmacy.
Garner (Providence & Moda)

Providence Health Plan and Moda Health are partnering with Garner. A one-pager on how to use Garner is available here

  • Garner is a tool that helps connect members with high-quality care.
  • Starting October 1, you can set up a Garner account and start searching for Top Providers.
    • Remember, repayment for any out-of-pocket expenses begins January 1, 2026, when the new plan year begins. 
  • Starting January 1, if you choose to see a Garner-approved provider, Garner will repay you for the costs of your visit. This includes your deductible, copay, and/or coinsurnance. It also includes prescriptions, labs, and other services ordered by the Top Provider.
    • You may be repaid up to:
      • $1,000 per year, if you have individual coverage
      • $2,000 per year, if you have family coverage

Dental Plans

Delta Dental Plans
  • Sinus lifts and bone grafting: Will be covered for dental implants.
  • Non-IV conscious sedation: Will be available for patients with developmental or intellectual disabilities.
Kaiser Dental
  • Orthodontia lifetime maximum will increase from $1,500 to $2,500.

Vision Plans

  • No plan changes. 

Optional Plans

  • PEBB will offer the same optional benefits to support important life changes. For 2026, the rates for these benefits will stay the same. 
  • Corestream: LegalEASE (legal services) and Kashable (personal loan program). Click here to login to Corestream. 

 

Classified Employees - Low Cost Plan Change in 2025

The typical cost share for core health benefits at OSU is a 95/5 split with OSU paying ninety-five percent (95%) and the employee paying five percent (5%). Where an employee has the opportunity to choose between two (2) medical plans and the employee enrolls in the least expensive PEBB medical plan available to them, OSU contributes ninety-seven percent (97%) to the cost of premiums for the coverage and the employee contributes three percent (3%).

In 2025, Kaiser Deductible will continue as the lowest cost plans in most areas in Oregon. This means that in many situations the lowest cost plan in the county in which you live or work will be the Kaiser and in some cases, Providence Choice or MODA Synergy plans. This is not something OSU or PEBB controls. Rates are set through the contracting process with the carriers by the PEBB Board.

What this means for you: Classified employees outside of the Kaiser Service area will be able to elect either Providence Choice or MODA Synergy as their least costly plan.

This may be important information from you to consider when you complete the Open Enrollment Process in October and make your plan choices. Employees may use the PEBB payroll deduction estimator to learn more about the lowest cost plan in the county in which they live or work.

Opt-Out or Decline? There is a difference.
  • Opt-out is a choice that results in PEBB membership. Employees that have other qualifying medical coverage can elect to opt-out of the PEBB medical enrollment and receive a cash payment in their monthly payroll checks. Employee’s MUST attest that all tax dependents have minimal essential health insurance coverage in order to qualify. To continue to opt out of medical benefits in 2026 you must:
    • Complete open enrollment between Oct. 1 and Oct. 31, 2025.
    • Attest that you still have other group (employer) coverage to receive the opt out incentive. If you don’t complete open enrollment you will remain without medical coverage and lose your monthly incentive.
  • Opt out is allowed for medical only and employees must attest to having other employer group coverage to be eligible. This means all current enrolled opt out employees need to complete open enrollment to attest regarding other coverage. If employees who have opted out previously do not attest, they will be removed from opt-out cash back and will be defaulted into a medical plan.
  • Decline is that an employee didn’t take an action to enroll in any PEBB option (including the opt-out cash payment option). You are not a PEBB member and receive no PEBB benefits.

2026 Medical Home Plan - Providence Choice

Remember to register your Medical Home with the Plan before you and any enrolled family members incur medical expenses in 2026.

To receive Medical Home level of benefits, you must choose a medical home within the plan network and notify Providence of your choice before accessing services. 

If you wish to keep your Medical Home the same you do not need to call and re-designate it.  It will remain the same.

2026 PCP 360 Plan – Moda Health

  • Members must choose and use a PCP 360 for each covered individual for in-network primary care services.
  • Members who already have a PCP 360 selected do not have to re-select a PCP 360 unless they would like to change who they see for primary care services.
  • To search for PCP 360 providers, visit: modahealth.com/PEBB and use Moda’s online Directory Find Care –  Here
    • Click "Find Care" to search for a provider by location, gender or language spoken. When you search, be sure to choose your network (Synergy) and select "PCP 360" from the "Provider type" menu. For assistance in finding a PCP 360, call toll-free at 844-776-1593, Monday through Friday, 7:30 a.m. to 5:30 p.m. Pacific Time. TTY users, please call 711.
  • Members can select their PCP 360 in their Member Dashboard (myModa) or by calling Moda at 1-844-776-1593, Monday through Friday, 7:30 a.m. to 5:30 p.m. Pacific Time. TTY users, please call 711.

Effective Dates, Verification of Changes, and Corrections to Enrollments

When are Changes Effective? – Changes made during Open Enrollment are effective January 1, 2026, except for policies and/or amounts that require the approval of underwriters (life/long-term care).

Verify that Changes Have Been Processed – Review your Earnings Statement to confirm that your enrollments and deductions are correct.

  • December 2025 Earnings Statement will reflect changes to Medical, Dental, HEM financial incentive, Life, Accidental Death, Short-term Disability, Long-term Disability, and Long-term Care. For policies that require approval of an underwriter the change will be on the Earnings Statement the month following approval (life/long-term care).
  • January 2026 Earnings Statement will reflect enrollment in the Flexible Spending Account(s) for 2026.

Corrections to Enrollments

  • If there is an error on your Earnings Statement, contact Employee Benefits immediately at 541-737-2805 or send an email to [email protected].
  • The Open Enrollment Corrections period ends February 28, 2026. If you did not complete Open Enrollment, then you are not eligible to enroll in or make corrections to a FSA.
  • The deadline for submitting affidavits for newly added individuals by affidavit is 11/7/25.

Flexible Spending Accounts (FSAs)

Are you planning to enroll or re-enroll in an FSA for 2026?

Flexible Spending Accounts (FSAs) are a way use pre-tax dollars to reimburse yourself for qualified health care, dependent care and commuting/parking expenses. If you want to enroll or re-enroll in an FSA for 2026, you will need to indicate this in the PEBB open enrollment process during October. You can learn more about FSAs on the ASI Flex website

  • All current FSA accounts will end on December 31, 2025. Remember that this money does not roll over into the next benefit year. You forfeit any funds that you do not use by the end of the grace period end date of March 15, 2026. The deadline for claiming any valid expenses is March 31, 2026. Employees can find a list of eligible FSA expenses on the ASI Flex website. This link takes you to a list with four separate tabs organized by FSA type including health care, dependent care, parking and transit.

Make sure you have selected the correct duration for your employment type:

  • Academic Year Employees 9 month – Salaried
    • Select a 10 month FSA
    • Select July and August as the months in which you don’t have a paycheck.
  • Academic Year Employees 9 month – Salaried on deferred pay (9 over 12)
    • Select a 12 month FSA
  • Academic Year Employees 9 month – Hourly
    • If your position will be on summer leave 06.16.2026 - 09.15.2026, select a 9 month FSA.
  • 12 month Employees – Salaried and Hourly
    • Select a 12 month FSA
  • Do you have split appointments: Academic Year 9 month 50% FTE and 12 month 50% FTE?
    • Select a 12 month FSA

FSA information:

  • Health Care FSA
    • Up to $3300 for plan year 2026
  • Dependent Care FSA
    • Childcare & eldercare expenses
    • Up to $7500 per household (IRS rule) for plan year 2026
  • Commute FSA/Fringe

Vaccine Clinics

Corvallis clinic dates and location:

IMPORTANT: Participants must bring their insurance cards to their appointments. If Fred Meyer does not have or cannot obtain correct billing information prior to the vaccine clinic, they will be unable to provide your vaccine(s) until they've received payment from your insurance provider, and this will cause a delay to your vaccine appointment. They do not accept cash or check at the clinic. 

Steps for Signing Up for a Flu and/or COVID-19 vaccine

Register using benefit code OREGONSUVAC25. The scheduler closed on Thursday, October 23 at 5:00 pm. Look for our next clinics in the fall of 2026!

  1. Visit link OSU Vaccine Clinic Sign-up and enter the benefit code (ALL CAPS): OREGONSUVAC25
  2. Select vaccines (flu and/or COVID-19)
  3. Select your appointment time on 10/28 and click "Continue."
  4. We strongly encourage participants to fill out the "Prescription Insurance" section. If Fred Meyer does not have or cannot obtain correct billing information prior to the vaccine clinic, they will be unable to provide your vaccine(s) until they've received payment from your insurance provider, and this will cause a delay to your vaccine appointment.

  5. Fill out the medical information and other patient details pages and click "Continue."
  6. Read and Acknowledge the Consent Form by clicking Yes at the bottom of the page and then "Continue."
  7. Enter the patient’s legal name and relationship to the responsible party on the following page and click "Continue."
  8. You are done! You should see a confirmed appointment and confirmation code on the webpage.

 

PEBB health plan members who do not wish to attend these clinics but are interested in a Flu shot can also consider these options:
Moda Health

PEBB members enrolled in Moda Health medical plans can visit any in-network pharmacy to receive a flu shot at no cost. Use Moda's Find Care tool to discover an in-network pharmacy near you or call your Moda 360 health navigator at 1-844-776-1594.

Providence Health Plan

PEBB members enrolled in Providence medical plans can visit any in-network pharmacy to receive a flu shot at no cost.

Kaiser Health Plan

Once again this year, PEBB members enrolled in a Kaiser Health Plan can attend the OSU Work Site Flu Clinics. For more information about other Kaiser Flu Shot optins, use the 2025 Flu Shot clinics locator

Additional Options
