Motor Vehicle History Checks

Overview

A motor vehicle history check is required for all employment and volunteer opportunities that require driving as part of the regularly assigned responsibilities.

Confidentiality

The university is committed to protecting the integrity and confidentiality of information gathered during the process. Motor vehicle history check details are not shared with hiring units/volunteer service units or any other unit that does not have a business need to know. If it is determined an individual is ineligible for a position, the hiring unit is only notified the individual is ineligible for the opportunity.

How to Request and Submit Motor Vehicle History

Individuals obtain their official/certified driving history directly from the DMV in the state that issued their driver’s license. The history must be for the past 36 months (three years).

Oregon license holders will request a Certified Court Print.

Most DMV offices will fax directly to OSU if it is requested they do so. Faxes should be sent to 541-737-7771, Attn: Background Check.

Mail hard copies to:

Oregon State University
University Human Resources
Attn: Background Check
1600 SW Western Blvd Suite 150
Corvallis, OR 97331-8578

This information is obtained at the cost of the individual. Please act on this as soon as possible. Delay in doing so may cause your background check process to take longer.
