Requesting Oregon Paid Family and Medical Leave (OPFML/PLO)

To request Oregon Paid Family and Medical Leave (OPFML/PLO), you will need to complete the application and submit supporting documentation. The supporting documentation required is outlined in the application form and can be found below. Forms are submitted via email to [email protected] once completed.
