Open Enrollment Summary & 2025 Plan Year Changes
PEBB open enrollment is October 1-31, 2024, and is not required this year.
What does that mean for you?
It means you must login to your PEBB portal if you need to make changes to any of the following:
- Change any of your coverages
- Add eligible dependents, remove dependents,
- Enroll or re-enroll in a flexible spending account (FSA)
- Change your Health Engagement Model (HEM) status
- Update your personal information
- Update your beneficiaries
You do not need to complete open enrollment if you are not making changes and do not need to re-enroll in flexible spending accounts. Open Enrollment is your once-a-year opportunity to make changes to your benefit packages without having experienced a qualifying life event or family status change (marriage, birth, divorce, loss/gain of other insurance, etc.).
The changes you make during Open Enrollment will be effective in the new plan year beginning January 1, 2025.
How do you complete open enrollment?
Employees can enroll online or use paper forms.
Go to: www.Pebbenroll.com to complete this process online.

Paper forms can be faxed, scanned and e-mailed or sent through mail.

Life Events Outside of Open Enrollment: If you incur or anticipate incurring a qualifying life event during Open Enrollment such as birth, marriage, divorce, contact Employee Benefits at 541-737-2805 or [email protected] in order to complete the Midyear Change process prior to completing Open Enrollment. If you incur a qualifying life event during the year, don’t wait until Open Enrollment to update your elections. Contact Employee Benefits or refer to our website on how to update you benefit elections. Visit the Midyear Changes to Health & Optional Benefits page for additional information.
Dependent Eligibility (required)
PEBB will request verification of eligibility for dependents added during Open Enrollment and throughout 2025 for qualifying Midyear Changes (birth, marriage, etc.).
What does this mean? If you add dependents during Open Enrollment or through a Midyear Change Form during 2025, PEBB will send you a packet of information. PEBB will ask you to confirm that each of your dependents are eligible. To complete the process, you will simply:
- Make copies of requested proof
- Mail, fax, or email the above to the PEBB Dependent Eligibility team. (NOTE: Email is not a secure method for sending documents.)
Read more about dependent eligibility on the Oregon Health Authority website. Make sure you have everything you need. Don't send anything now, wait until you receive your letter from PEBB later this year. NOTE: If you do not respond to PEBB's letter, PEBB will remove the dependents added to your coverage.
Surcharges and Optional Life Rates
- Tobacco Surcharge - $25/month if employee uses tobacco + $25/month if enrolled spouse/domestic partner uses tobacco
- Spouse/Domestic Partner Surcharge - $50/month if spouse/domestic partner waives coverage in another employer’s group plan (non-PEBB plan) in favor of coverage through PEBB.
- Tobacco User Life Insurance Rates – The rates for optional employee life and optional spouse/domestic partner life insurance is higher for members that have used tobacco within the past 12 months
- OEBB/PEBB double coverage Surcharge - You will have to pay a monthly $5 surcharge if you are an active full-time employee and:
- Someone in your family is covered as a member under their own PEBB or Oregon Educators Benefit Board (OEBB) plan, and
- That person is covered as a dependent (spouse, partner or child) on your PEBB medical plan.
2025 Plan Changes
Medical Plans
General/All Medical Carriers:
Doula services: Doula services will now be available. Services include eight pre- and post-natal care visits, plus delivery.
MODA Plans
- Teladoc: Teladoc is a virtual primary care provider that will be part of Moda’s Coordinated Care Model (CCM) network. This expands access to primary care for all PEBB members, especially those in rural Oregon and out of state.
- Mighty: Mighty offers a holistic approach to weight management. It focuses on nutrition behavior changes. It also helps reduce pain and improve mobility and function. (Mighty will be offered at no cost as an alternative to WeightWatchers®.)
- Nutritional therapy: Appointments will be covered at the standard cost-sharing without requiring a diagnosis.
- Fertility Services will now include:
- Creating embryos using the intended parent’s sperm for same-sex male couples.
- Coverage for those who have undergone voluntary sterilization.
- Cryopreservation (egg or sperm freezing) for members undergoing gender affirming care when medically necessary.
- Coverage through Kindbody, including comprehensive care services and concierge patient advocacy.
- Costco pharmacy: Members will be able to fill $0 cost-share value tier prescriptions at Costco. This program will provide a six-month supply. A membership is not required.
- Covered drugs: The plan will no longer cover certain over-the-counter drugs.
Kaiser Plans
- Fertility Services will now include:
- Cryopreservation (egg or sperm freezing) for members undergoing gender affirming care when medically necessary.
- Storage of cryopreserved tissue when medically necessary.
- Omada: Omada is a weight management program. (This program will be offered at no cost as an alternative to WeightWatchers®.)
- Traditional HMO pharmacy benefits: The costs for drugs will increase. ‒ Generic drugs: The copay will increase from $1 to $5.
- Preferred brand drugs: The copay will increase from $15 to $25.
- Non-preferred brand drugs: The cost-share will increase from a $15 copay to 50% of the cost, up to $100.

Providence Plans
Medical Plan Updates
Generic medications: Members will be required to use generic medications instead of brand-name medications when available.
- Alopecia areata medications. Members with severe alopecia areata can request an exception to the exclusion of hair growth medications.
- Rx Savings Solutions. This program will be available to identify potential cost-saving opportunities for members.
Covered drugs: The list of “value tier” drugs will be updated. This list includes low-cost generic drugs used to treat the most common chronic conditions. Also, the plan will no longer cover certain over-the-counter drugs.
- Fertility Services will now include:
- Coverage for members who have undergone voluntary sterilization.
- Cryopreservation (egg or sperm freezing) for members undergoing gender affirming care when medically necessary.
- A new model for fertility benefits. Members will receive up to two advanced reproductive technology (ART) cycles each year if the first cycle doesn't work. Each cycle includes all necessary tests and the latest technology the member's provider decides is needed.
- Progyny: Fertility and family-building benefits, including comprehensive care services and concierge patient advocacy, will be available through Progyny.
- Mighty: Mighty offers a holistic approach to weight management. It focuses on nutrition behavior changes. It also helps reduce pain and improve mobility and function. (Mighty will be offered at no cost as an alternative to WeightWatchers®.)
Dental Plans
Delta Dental Plans
- Services for those with intellectual or developmental disabilities. Covered services will be expanded to include visits before the first treatment, two additional cleanings per year, silver diamine fluoride for cavities, and care enhancements for those with sensory or behavioral challenges.
Kaiser Dental
- Nightguards: The cost for occlusal nightguards will increase from 10% to 35%. Nightguards will be covered once every five years.
- Mouthguards: The cost for athletic mouthguards will increase from 10% to 35%.
Vision Plans
Contact lens: The allowance will increase to $225 every calendar year.
Optional Plans
- PEBB will continue to offer other income protection benefits in 2025. There will be no change to rates for disability insurance, life insurance, and AD&D insurance.
- The cost for long-term care insurance may increase during the year. Costs are based on the coverage options the member chooses and the place of residence used.
Classified Employees - Low Cost Plan Change in 2025
The typical cost share for core health benefits at OSU is a 95/5 split with OSU paying ninety-five percent (95%) and the employee paying five percent (5%). Where an employee has the opportunity to choose between two (2) medical plans and the employee enrolls in the least expensive PEBB medical plan available to them, OSU contributes ninety-seven percent (97%) to the cost of premiums for the coverage and the employee contributes three percent (3%).
In 2025, Kaiser Deductible will continue as the lowest cost plans in most areas in Oregon. This means that in many situations the lowest cost plan in the county in which you live or work will be the Kaiser and in some cases, Providence Choice or MODA Synergy plans. This is not something OSU or PEBB controls. Rates are set through the contracting process with the carriers by the PEBB Board.
What this means for you: Classified employees outside of the Kaiser Service area will be able to elect either Providence Choice or MODA Synergy as their least costly plan.
This may be important information from you to consider when you complete the Open Enrollment Process in October and make your plan choices. Employees may use the PEBB payroll deduction estimator to learn more about the lowest cost plan in the county in which they live or work.

Opt-Out or Decline? There is a difference.
- Opt-out is a choice that results in PEBB membership. Employees that have other qualifying medical coverage can elect to opt-out of the PEBB medical enrollment and receive a cash payment in their monthly payroll checks. Employee’s MUST attest that all tax dependents have minimal essential health insurance coverage in order to qualify. To continue to opt out of medical benefits in 2025 you must:
- Complete open enrollment between Oct. 1 and Oct. 31, 2024.
- Attest that you still have other group (employer) coverage to receive the opt out incentive. If you don’t complete open enrollment you will remain without medical coverage and lose your monthly incentive.
- Opt out is allowed for medical only and employees must attest to having other employer group coverage to be eligible. This means all current enrolled opt out employees need to complete open enrollment to attest regarding other coverage. If employees who have opted out previously do not attest, they will be removed from opt-out cash back and will be defaulted into a medical plan.
- Decline is that an employee didn’t take an action to enroll in any PEBB option (including the opt-out cash payment option). You are not a PEBB member and receive no PEBB benefits.

2025 Medical Home Plan - Providence Choice
Remember to register your Medical Home with the Plan before you and any enrolled family members incur medical expenses in 2025.
To receive Medical Home level of benefits, you must choose a medical home within the plan network and notify Providence of your choice before accessing services.
If you wish to keep your Medical Home the same you do not need to call and re-designate it. It will remain the same.

2025 PCP 360 Plan – MODA Health
- Members must choose and use a PCP 360 for each covered individual for in-network primary care services.
- Members who already have a PCP 360 selected do not have to re-select a PCP 360 unless they would like to change who they see for primary care services.
- To search for PCP 360 providers, visit: modahealth.com/PEBB and use Moda’s online Directory Find Care – Here
- Click "Find Care" to search for a provider by location, gender or language spoken. When you search, be sure to choose your network (Synergy) and select "PCP 360" from the "Provider type" menu. For assistance in finding a PCP 360, call toll-free at 844-776-1593, Monday through Friday, 7:30 a.m. to 5:30 p.m. Pacific Time. TTY users, please call 711.

Exemptions from the HEM Health Assessment
An EXEMPTION from the HEM Health Assessment can be requested if:
- Member identifies as Transgender (the health assessment does not take into account the needs of the member).
- Member is enrolled in Hospice or extended hospital stays preventing them from completing their HEM action requirements
- Member is out of the country from their plan year coverage effective date and their HEM action requirements.
- Medical condition(s) that would not allow an individual to complete the health assessment and/or the health activities
- Exemption Form
Health Engagement Model (HEM)
This year's Open Enrollment is "passive" (or non-mandatory) so HEM participation will look a bit different. Review the detailed Decision Chart to see what actions you need to take during Open Enrollment.
If you're participating in HEM in 2024 and:
- You aren't making any benefit changes for 2025, you don't have to do anything. Your enrollment will automatically roll over to 2025. You do not need to take the Health Assessment in 2024. You'll continue to receive the lower medical plan deductible and incentive.
- You want to change your medical plan for 2025, you must enroll in the new medical plan during Open Enrollment. Be sure to state you plan to participate in HEM. You do not need to take the Health Assessment in 2024. You'll continue to receive the lower medical plan deductible and incentive.
If you don't currently participate in HEM and want to participate in 2025, you must:
- Enroll in a PEBB medical plan for 2025 between Oct. 1 and Oct. 31, 2024. Be sure to state you plan to participate in HEM.
- Take your health assessment on your current medical carrier's website between Sept. 1 and Oct. 31, 2024: Health assessments done outside of these dates will not be accepted. Proof of completion is not required but we do recommend that you take a screenshot of your health assessment summary or print your completion certificate.
- Complete two health actions during the plan year. You don't have to report them. You just need to let us know you did them at Open Enrollment next year.
Employees without medical coverage
(Opt-out, decline members, or employees who have never enrolled:)
- Normally to participate in HEM, members take a health assessment on their current medical plan's website. Even though you are not enrolled in a medical plan this year you can still participate in HEM though. You just need to enroll in medical coverage during Open Enrollment. You won't take a health assessment this year. You will take one next Open Enrollment when you can take it on your medical carrier's website.
To participate in 2025:
- Enroll in a PEBB medical plan for 2025 between Oct. 1 and Oct. 31, 2024. Be sure to state you plan to participate in HEM.
- Complete two health actions during the plan year. You don't have to report them. You just need to let us know you did them at Open Enrollment next year.
New Hires:
- To be eligible for HEM, your benefits must be in effect by Oct. 1. If your benefits are effective Nov. 1 or later, you are not eligible for HEM for the upcoming plan year. You can participate in HEM next Open Enrollment.
- HEM is an Open Enrollment benefit only.
- New employees not eligible for HEM do not receive the monthly incentive but are enrolled in the lower deductible plan.
- Health Engagement Model (HEM) PARTICIPATION in the 2025 HEM program:
- Enroll in the HEM program during Open Enrollment, October 1 – October 31, 2024.
- Complete of the online HEM health assessment between September 1 and October 31, 2024. You MUST complete the online health assessment.
- An Exemption form the Health Assessment may be claimed in certain circumstances, see Exemptions from the HEM Health Assessment.
- Complete two health related activities during 2024
- Health Activities are individualized and may include getting a physical, going to the dentist, starting and/or continuing an exercise program, walking at lunch time, and eating more fruits and vegetables.
- For employees that use tobacco, one activity will involve trying to quit.
- For employees that are overweight, one activity will involve trying to lose weight.
- Health Actions FAQs
- Employees will attest to completing the requirements during Open Enrollment in 2024 (to be eligible to participate in 2025).
- Newly eligible employees hired in October, November and December 2024, and during plan year 2025, are not eligible for HEM participation. They will not receive a monthly incentive and are only allowed to enroll in the standard annual deductible medical plans. New hires from September 2024 who did not enroll until October with coverage effective November 1st also are not eligible for the HEM.
- Plan early to complete your Health Assessment. You may need to have your password reset which can take time.

Please Note: The HEM credit shows on OSU earn statements as -17.50 (a credit to the employee).
Effective Dates, Verification of Changes and Corrections to Enrollments
When are Changes Effective? – Changes made during Open Enrollment are effective January 1, 2025, except for policies and/or amounts that require the approval of underwriters (life/ long-term care).
Verify that Changes Have Been Processed – Review your Earnings Statement to confirm that your enrollments and deductions are correct.
- December 2024 Earnings Statement will reflect changes to Medical, Dental, HEM financial incentive, Life, Accidental Death, Short-term Disability, Long-term Disability, and Long-term Care. For policies that require approval of an underwriter the change will be on the Earnings Statement the month following approval (life/long-term care).
- January 2025 Earnings Statement will reflect enrollment in the Flexible Spending Account(s) for 2025.
Corrections to Enrollments
- If there is an error on your Earnings Statement contact Employee Benefits immediately at 541-737-2805 or send an email to [email protected].
- The Open Enrollment Corrections period ends February 28, 2025.If you did not complete Open Enrollment, then you are not eligible to enroll in or make corrections to a FSA.
- The deadline for submitting affidavits for newly added individuals by affidavit is 11/7/24
- Forms for Corrections are located on the PEBB Vendor/Carrier Forms page by searching for the Open Enrollment and Corrections Form.
Flexible Spending Accounts (FSAs)
Are you planning to enroll or re-enroll in an FSA for 2025?
Flexible Spending Accounts (FSAs) are a way use pre-tax dollars to reimburse yourself for qualified health care, dependent care and commuting/parking expenses. If you want to enroll or re-enroll in an FSA for 2025, you will need to indicate this in the PEBB open enrollment process during October. You can learn more about FSAs on the ASI Flex website.
- All current FSA accounts will end on December 31, 2024. Remember that this money does not roll over into the next benefit year. You forfeit any funds that you do not use by the end of the grace period end date of March 15, 2025. The deadline for claiming any valid expenses is March 31, 2025. Employees can find a list of eligible FSA expenses on the ASI Flex website. This link takes you to a list with four separate tabs organized by FSA type including health care, dependent care, parking and transit.
Make sure you have selected the correct duration for your employment type:
- Academic Year Employees 9 month – Salaried
- Select a 10 month FSA
- Select Jul and Aug as the months in which you don’t have a paycheck.
- Academic Year Employees 9 month – Salaried on deferred pay (9 over 12)
- Select a 12 month FSA
- Academic Year Employees 9 month – Hourly
- If your position will be on summer leave 06.16.2025 - 09.15.2025 then select a 9 month FSA.
- 12 month Employees – Salaried and Hourly

Information you will need to register for the Flu or COVID Vaccine:
- The benefit code is case sensitive (all caps): OSUVAC24 and will be needed to sign up for this event
- We will be offering FluBlok Trivalent vaccine AND this season’s 2024-2025 COVID vaccine (Pfizer’s Comirnaty or Moderna’s Spikevax depending on product availability). Both flu and COVID vaccine will be allocated based on first-come, first-serve availability. COVID supply availability is limited.

- Unfortunately we do not accept Express Scripts, ESI, or Cigna, as well as subsidiaries or partners of Cigna/Express Scripts.
- OSU will notify you by email prior to your appointment if we were unsuccessful billing your pharmacy insurance.
- Please bring all insurance cards with you to your appointment
- Do not sign up under the Corvallis Fred Meyer location.
- You have until September 26th at 5pm to sign up for the October 3 vaccine clinic. There will be an additional clinic on 10/17. Sign-up availability and times will be provided at a later date.
Steps for Signing Up
- OSU Vaccine Clinic Sign-up
- Enter the benefit code (ALL CAPS): OSUVAC24
- Enter ‘Vaccines,’ ‘Corvallis, Oregon,’ and ‘10/3/2024’ then click Find Appointment
- For the following page, we are ONLY offering Flu and COVID 12 years of age and older (we will not have pediatric COVID vaccine available). Select which vaccine(s) you’d like to receive and click Continue at the bottom of the page
- Select your appointment time and click continue.
- Fill out the Patient Eligibility Section. Choose No for ‘Does the patient have a prescription,’ then Continue.
- Then Continue again on the following page to confirm your appointment and information.
- Fill out the Medical Information page and click Continue
- Fill out the Patient Details page and click Continue. Select No for ‘Would you like to add your primary health care provider’s information?’ (all vaccines administered will be upload into the State’s Vaccine Immunization Information System Registry).
- Read and Acknowledge the Consent Form by clicking Yes at the bottom of the page and then Continue.
- Enter the patient’s legal name and relationship to the responsible party on the following page and click Continue.
- Done!
PEBB health plan members who do not wish to attend these clinics but are interested in a Flu Shot can also consider these options:
Moda Health
PEBB members enrolled in Moda Health medical plans can visit any in-network pharmacy to receive a flu shot at no cost. Use Moda's Find Care tool to discover an in-network pharmacy near you or call your Moda 360 health navigator at 1-844-776-1594.
Providence Health Plan
PEBB members enrolled in Providence medical plans can visit any in-network pharmacy to receive a flu shot at no cost.
Kaiser Health Plan
PEBB members enrolled in a Kaiser Health Plan are able to attend the OSU Work Site Flu Clinics. Kaiser has also partnered with Albertsons/Safeway and Fred Meyer for employer-sponsored flu shot clinics this year. You may find more information here: 2024 Worksite Flu Shot Clinics (kp.org)

- Vaccine price for uninsured and ineligible participants:
- Regular dose: $35.50
- High dose $64.00

- Minimum billing: Negotiable. At time of confirming/contracting, a minimum requirement may be presented. It would not exceed 30% participation requirement or a potential one-time fee of $150.
- Vaccine price for uninsured and ineligible participants: Tiered rates and discounts are available and are based on number of estimated participants at the time of confirming/contracting for services.
